New Patients
Welcome to Minnesota Dermatology Associates
We’re glad you’re here. We provide comprehensive, evidence-based medical dermatology care for patients ages 6 and older, with a focus on personalized care in an environment where you feel heard, informed, and never rushed.
What to Expect at Your First Visit
Your first appointment gives us an opportunity to understand your concerns and medical history. Your provider will review your medications, allergies, relevant medical history, and the skin concerns you would like addressed.
Depending on the reason for your visit, your appointment may include:
A focused or full-body skin examination
Evaluation of new or changing moles or skin lesions
Diagnosis and treatment of skin conditions
Skin biopsies when medically indicated
Cryotherapy (freezing treatment) for certain lesions
Discussion of treatment options and a personalized care plan
We encourage questions and want you to leave your appointment understanding your diagnosis, treatment options, and next steps.
Before Your Appointment
Please bring:
A valid photo ID
Your current insurance card
A current list of medications and allergies
Any relevant medical records or pathology reports
If you are scheduled for a full-body skin examination, we recommend avoiding heavy makeup, nail polish, and self-tanner whenever possible, as these may make portions of the examination more difficult.
Please arrive 10–15 minutes before your scheduled appointment to allow time for check-in.
Insurance
Minnesota Dermatology Associates participates with many major insurance plans, including:
Medicare
Blue Cross Blue Shield
Medica
UnitedHealthcare
HealthPartners
Tricare
Cigna
Sanford Health Plan
Aetna
Humana
If you do not see your insurance listed, please contact our office. Insurance participation is subject to change, and coverage varies by individual plan. We recommend confirming your dermatology benefits with your insurance company prior to your appointment.
Patients are responsible for any applicable copays, deductibles, coinsurance, and non-covered services.
Patient Forms
New patient forms will be available through our secure patient portal prior to your appointment. Completing your forms before your visit helps us make the most of your appointment time.
Payment Policy
Patients using insurance are required to keep a valid credit or debit card on file. Copays and other amounts due at the time of service may be collected at your appointment. After your insurance processes your claim, your card on file may be charged for any remaining patient responsibility, including deductibles, coinsurance, or non-covered services, in accordance with our financial policy.
A 3% processing fee applies to credit card payments. Debit card payments are not subject to this fee.
For patients choosing our self-pay option, payment is due in full on the day of service.
Cancellation & No-Show Policy
We understand that plans can change. If you need to cancel or reschedule your appointment, please notify our office at least 24 business hours in advance so we can make that appointment available to another patient.
Appointments canceled with less than 24 business hours' notice, as well as missed appointments, may be subject to a $25 fee.
